Statement of Continuing Pharmacy Education Credit:
The University of Illinois at Chicago College of Pharmacy is accredited by the Accreditation Council for Pharmacy Education (ACPE) as a provider of continuing pharmacy education. Pharmacists have the opportunity to earn a maximum of 10.0 contact hours (1.0 CEUs) for participating in all knowledge-based activities at this conference (5 live hours with the option to earn additional 5 hours with the completion of 5 homestudies). Attendance at each knowledge-based activity, completion of the self-assessment questions on the evaluation form, and submission of NABP eProfile ID and MMDD are prerequisites for receiving continuing pharmacy education credit(s). All ACPE credit awarded to participants must be uploaded to CPE Monitor™ within 30 days of the activity date, so pharmacists are required to provide their e-Profile ID and date of birth (MMDD) at the time of the activity. Statement of Continuing Nursing Education Credit:
UI Health is approved as a provider of nursing continuing professional development by the Ohio Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation. (OBN-001-91).
In regard to successful completion and contact hours being awarded: 5.0 Continuing Nursing Education contact hours will be awarded to participants who attend the entire duration of the activity, and complete and return the evaluation form at the end of the activity. Advanced practice nurses may count a total of 5.0 contact hours (Adult Track) or 5.0 contact hours (Pediatric Track) for the pharmacotherapeutic educational requirement.
In addition to the live program, participants may earn an additional 5 hours with the completion of 5 homestudies.
